Notes

Reference Time is Central European Time (CET)

The BVG ticket is a pass that allows for free transportion on all public transport within the AB zone (which covers most of Berlin) as well as from and to the airports. It is valid from September 26 to 30.

Attendance of social functions depends on availability.
The organizers reserve the right to change prices and/or services.


Additional Information for Registration

Availability

The congress venue as well as the social function locations are limited in their capacity. The organizers reserve the right to close the registration as soon as the maximum capacity is reached. If only parts of the registration are not available (e.g.a social function or tour), the registration will be processed without that part. Refunds will be given for the bookingitems not available.

Group Registration

For group registrations (>10 participants) please note that we will not accept any group reservations without the individualmailing addresses of the participants. You will have to provide the individual mailing address for each participant. If you would like to register as a group, please contact the ISPAD 2007 Conference Secretariat.

Reduced Fees

By selecting Student, Nurse, Young Physician or Educator you confirm that you will prove your status by faxing (Fax: +49- 30-24603-200) a photocopy of the student ID card or a letter verifying that you work in the hospital as a trainee or nurse or a proof of full-time enrolment at an accredited university or college within 2 weeks.

Media Registration

To register as a media representative, please complete and send in the registration form enclosed in this 2nd Announcement or register online at www.ispad2007.com. Please check the appropriate box on the registration form. There is no fee for accredited media representatives. To register as a media representative for the conference, proof of accreditation is required. It can either be in the form of a photocopy of official press credentials or a letter verifying your assignment to the conference on the stationary of the media organization for which you work. Freelance journalists wishing to cover the conference must submit copies of three by-lined articles in addition to an official letter assignment. Newsletter writers and editors must submit three issues of their newsletter, containing at least one by-lined article written by the applicant. All credentials will be verified by the conference staff.

Letter of Invitation

Individuals requiring an official letter of invitation in order to obtain a visa and authorization to attend the conference should contact the ISPAD Conference Secretariat. The letter of invitation does not financially obligate the conference organizers in any way. All expenses incurred in relation to the conference are the sole responsibility of the participant.

Letter of Confirmation

Registration to the conference will be confirmed by a letter of confirmation as soon as the fully completed registration forms as well as the payment have been received by the ISPAD Conference Secretariat. Participants must present this confirmation letter at the registration counter as proof of their registration.

Changes

Notification of changes (name changes, address changes etc.) must be made in writing and sent to the ISPAD Conference Secretariat. A handling fee of € 30 will be charged for any name change made to an already received conference registration. A newly completed registration form for the substitute participant has to be sent to the ISPAD Conference Secretariat. Changes will be accepted in writing only until September 14, 2007. After this date, all changes must be carried out on-site.

 

 
Cancellation of Registration

Notification of cancellation must be made in writing and sent to the ISPAD Conference Secretariat with a reference to a bank account, where a possible refund may be remitted to. A cancellation fee will be charged depending on the date of the cancellation. If the cancellation has been received before June 2, 2007 a cancellation handling fee of € 30 will apply. For cancellations received between June 2 and September 14, a cancellation handling fee of 50% of the total registration fee will apply. If cancellations are received later than September 14, registration fees will not be refunded. Credit will not be given for unattended events or early termination of attendance. All refunds will only be issued after the conference.

Lost Name Badge

The name badge must be worn at all times during the 33rd Annual Meeting of the International Society for Pediatric and Adolescent Diabetes. Access to the Conference facilities will not be granted without a proper name badge. If a delegate loses, misplaces or forgets his name badge, a handling fee of € 30 will be charged for a new name badge.

On-Site Check-in Procedures

All participants are required to personally check in at the appropriate registration counter at the bcc to receive their badge and the conference bag, before attending any conference session or event. The on-site registration counters will be open between Wednesday 26 and Friday 28.
